Functional Capacity Assessments
A functional capacity assessment includes a face-to-face consultation with you and your supports combined with the development of a report that informs NDIS and your health care team how you are managing your day-to-day life. A functional capacity assessment is aimed at documenting the individual impact of your disability, whether that be musculoskeletal, neurological, cognitive, psychosocial, sensory, or otherwise. Our FCA generally involves identifying how an individual is coping within their living environment, completing observations, collaborating with the participants stakeholders, and compiling a detailed report aimed at maximising your NDIS plan.
Home Modifications
Minor home modifications can be applied to your home to support improved independence, safety and mobility. We are able to provide expert advice, assessment and recommendation for your home to determine whether home modification solutions may be suitable for you. This may include, but is not limited to major bathroom modifications, kitchen modifications, installation of grab rails in a shower, and internal and external hand rails next to steps, installation of non-slip surfaces.
Supported Independent Living Assessments
A Supported Independent Living (SIL) Assessment involves the OT providing a comprehensive assessment of your functional status and the support levels you require to live as independently and safely within your home with their everyday care needs been met.
Specialised Disability Accommodation Assessments
Specialist Disability Accommodation (SDA) is a type of housing or accommodation that can be fitted with specific modifications or assistive technologies that cater for an extreme functional impairment or very high support needs. SDA Assessment involves an extensive evaluation of your current and future needs and provides a recommendation to NDIS regarding your eligibility for specialist disability accommodation and the specific environmental features the participant requires for optimal independence and safety. This can include specific recommendations around wheelchair accessible homes, installation of ceiling hoists, use of automation technology or robust features to aid safety.
Equipment Prescriptions
Assistive technology can be equipment, aids or devices that can make your life easier, safer, or support you to achieve greater independence. Whether your goal is to cut food independently, feel safe while showering, or get around with seated or standing mobility devices, our therapists are able to provide expert opinion and assessment to ensure this outcome is possible for you. Examples may include, but are not limited to power / manual wheelchairs, mobility scooters, bedding equipment, kitchen-related products, transfer-aides, and general safety items for the home.
End Of Plan Assessments
An End of Plan Report is an important document that includes the details of a functional capacity report with the addition of your progress (if applicable) to date and recommendations on future needs and supports. These are submitted to NDIS for consideration when they are renewing your NDIS plan.
Daily Living Skills & Therapy Support
The goal of daily living therapy supports is to help you improve your independence and enable participation in daily and practical activities. This covers communication, mobility, movement, personal care, interpersonal interactions and community living. Occupational Therapy can help you develop your functional skills to improve your participation and independence.
